Palliative Care Consulting Group, LLC (PCCG) was established in January of 2000 to provide clinical pharmacy consulting in the area of pain and symptom management. Although the original focus was on patients with life-limiting illnesses, PCCG also consults with physicians and patients with chronic non-malignant pain and other chronic illnesses.
Over the past five years, PCCG has worked with over 100 hospice and palliative care programs nationally and has provided consultation with physicians on drug therapy management in patients who are not terminally ill. |

Palliative Care Consulting Group offers a variety of clinical services. We strive to meet the diverse needs of each client and their environment.
· The PCCG clinical pharmacists specialize in patient specific medication therapy management for all types of chronic pain and chronic illness as well as symptoms that relate to life-limiting illness. They serve as a support to the physician and other members of the health care team by providing drug therapy and dosing recommendations, monitoring of therapeutic effect, pharmacotherapeutic plan of care coordination and documentation.
· The PCCG pharmacists are trained in obtaining and evaluating medical and drug literature in order to provide clinically sound, comprehensive drug information services.
· The PCCG pharmacists use each consult as an opportunity to provide education and collaborative, proactive problem solving skills to the nurses they work with related to therapeutic interventions. The increased level of knowledge empowers the nurses and enhances communication with physicians, patients and caregivers.
· PCCG continually develops, updates and refines tools for providing quality and consistent patient care such as preferred drug lists, algorithms, protocols, patient information sheets and evidence-based physician information.
· The PCCG Pharmacists are actively involved with student and post-graduate training through their affiliations with The Ohio State University and Ohio Northern University. This involvement continues to provide opportunities for research and innovative, collaborative pilot programs that ultimately benefit the clients whom they serve.
· PCCG has partnered with experts in patient safety and healthcare technology to provide continuous improvement of services.
