Medication Therapy Management Customized clinical support for your staff and your mission
In partnership with your hospice, our clinical services help you improve patient care outcomes and elevate your staff’s level of clinical knowledge by providing patient-specific drug therapy expertise while assisting you with the critical tasks of lowering your overall medication costs and increasing the quality of patient care.
In addition, our programs are designed to support the patient medication profile review requirements and data collection efforts of the new Medicare Conditions of Participation (CoPs). Several options are available and our clinical staff will work with you to develop the program that best meets the needs of your hospice while optimizing your local resources. |

HospiScript offers clinical pharmacist services including consultations by telephone, written patient drug profile reviews, symptom management consultation, prior authorization assistance, educational programs, drug information, reporting and a variety of other clinical products and services. In our patient-specific clinical reviews, HospiScript pharmacists will assess each patient profile and communicate the results of the review back to you. The pharmacist will also review for: · Effectiveness of the drug therapy; · Adverse drug-effects; · Actual or potential drug interactions; · Duplicate drug therapy; · Drug therapy associated with lab monitoring needs; and · Potential cost effective therapeutic alternatives.
The pharmacists will make recommendations on which medications are related to the patient's terminal diagnosis and related conditions and should be covered under the hospice benefit. If requested as part of your program, our pharmacists will also participate in the prior authorization approval process for non-formulary medications. HospiScript also offers a variety of clinical services so whether you have the resources in house, or need full clinical oversight, we have the products and solutions to ensure the compliance in a manner that best meets your hospice's needs.
